Overview
The Ohio Hepatitis Academic Mentorship Program (OH-HAMP) is designed to train primary care providers throughout the state using an in-person and telementoring-based approach. OH-HAMP’s goal is to improve the health of individuals in underserved communities by building a primary care workforce trained by experts to screen, diagnose, treat, cure and follow persons with chronic hepatitis C. OH-HAMP primary care providers will receive training on hepatitis C epidemiology, diagnosis, management, curative treatment, follow-up and prevention from disease experts. Thus, primary care providers throughout the state will be equipped with the skills needed to treat patients with hepatitis C in their local communities. This will increase the number of individuals who are identified and cured and help to reduce the number of Ohioans who can transmit hepatitis C to another person. OH-HAMP is a free, easily accessible training program that will be available to providers across the state. It will meet the needs of both primary care providers interested in providing hepatitis C care and current Ohio Medicaid and private payor requirements. OH-HAMP is designed to build (educate and empower) a mentorship platform that is accessible to primary care providers caring for high risk individuals in every part of Ohio, especially in rural, low resource settings.


Objectives
At the conclusion of this activity, learners will be able to:

  1. Demonstrate knowledge of hepatitis C epidemiology, screening, diagnosis, management, curative treatment, follow-up and prevention.
  2. Describe strategies to identify persons with hepatitis C infection.
  3. Describe the increase of hepatitis C infection related to the injection drug epidemic in Ohio.
  4. Show how to counsel patients about transmission risks.
  5. Demonstrate understanding of the latest treatment options for hepatitis C infection
  6. Identify testing procedures for perinatal transmission of hepatitis C: screening, testing, and treatment of infants born to mothers with chronic hepatitis C infection.
  7. Describe the infrastructure necessary to support hepatitis care and treatment in primary care settings.
  8. Demonstrate understanding of the recent and evolving changes to Ohio’s Medicaid program and how these changes impact hepatitis C treatment in the state.

Accreditation
This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of the University of Toledo and Ohio Hepatitis Academic Mentorship Program. The University of Toledo is accredited by the ACCME to provide continuing medical education for physicians.

The University of Toledo designates this activity for a maximum of 8.00 AMA PRA Category 1 Credit(s)TM .  Physicians should claim only the credit commensurate with the extent of their participation in the activity.
